Creating Facilities

  1. Sign into PnC Web App

  2. Navigate to PnCAdmin → Scheduling → Facilities

  3. Click +Add

  4. The facility name is typically “Agency Abbreviation” Facility Name (ASU Facility)

  5. Facility Short name is the same as the Facility name

  6. Time offset hours is 0 for our cases

  7. Set the Agency Division to the agency this facility will belong to

  8. Fill in the address of the address of the ordering physician provided in the intake form or found here: https://docs.google.com/spreadsheets/d/1lpx3LY7nF6QFj9JkGh1wsWqNMIrgTDC52IaX1LjND6o/edit#gid=468689145

  9. Nothing under the Public Health Reporting section.

  10. Nothing under the Optional Facility Billing Address Override

  11. Set the CLIA number at the very bottom to be the ABCTL CLIA number of 03D2180875
